How to use the new Testing Center Instructor Request tool

The new online Instructor Request tool makes it quick and easy to request proctored exam services at FSU's Testing Center.

When using this tool, please keep in mind:

  • We strongly recommend designating an alternate contact for your course. 
    It is very important to have an alternate contact listed, such as TA(s) or co-instructor(s), for your exams. This helps to ensure a faster resolution in case any issues arise while your students are taking the exam at the Testing Center.
  • If you need to change any exam request information after your exam has been scheduled, please contact the Instructor Request Administrator at the Testing Center.
    You will not be able to adjust exam request information in the instructor request tool at this point.

How to request proctored exam services for your course

  1. Login to the Instructor Request tool.
  2. Click the Manage option in the banner at the top of the page, then select the term for which you want to make a proctored exam request.
  3. On the Manage Requests page, click the Add Course button.
  4. If you are an instructor of record, you may see a list of the courses and sections you are teaching. Add the course section(s) for which you want to request exams at the testing center by checking the desired course section(s) box(es), and then press the submit courses checked button.
    Keep in mind: You can add multiple sections at once if all the sections you are adding have the same exam information and exam window(s). However, if the exam information and windows are different for your course sections, you will need to fill out a separate request for each section.

    If you are not the instructor of record, or are a TA requesting an exam on behalf of the instructor of record, then you will need to manually add the course in order to request exam services for that course (skip down to "manually add course" instructions below).
  5. On the Manage Requests page, locate the course for which you would like to request proctored exam services. Click the Course Actions button for the course you would like to add an exam to. Select Add Exam.
  6. In the window that appears, fill out the exam information, including title, format, length, start and end dates, and any software that students will need. Then, select Next.
  7. Another window will appear prompting you to specify any exam aids you wish students to use and any special instructions for the exam. Select your preferences and type in any special instructions, then click the Submit button.
  8. You should now see the proctored exam window you just created listed on your Manage Requests page.

How to include a formula or reference sheet

If you opted to include a formula or reference sheet for your students to use, click on the exam title to go to the Exam Details page. On the Exam Details page, select Actions > Edit Exam Material.

Use the Choose File option to locate your formula sheet file on your computer, and then press Upload File to add it to your exam request.

Acknowledging the scheduled dates for your exam(s)

Once your proctored exam services request has been scheduled by the testing center, you will receive an email notification alerting you to the scheduled dates. You must acknowledge the scheduled exam dates and notify your students of the exam dates so they can sign up for a slot in Register Blast.

You can acknowledge the scheduled dates two ways:

  • Follow the link in the email alert you receive
  • Log in to the Instructor Request Tool and click on the bell icon next to your name in the upper right corner of the screen. Then, select the exam acknowledgement message:

    Check the box to the right of the exam you wish to acknowledge and then click the Acknowledge button.

How to manually add a course

  1. Click the Add Course button on the Manage Requests page.
     
  2. On the Add Course page, click the Didn't see your course? Add here link.
  3. Type in the course prefix and number with no spaces, e.g., ENC1101. Then, press the Search button.
  4. Select the course section(s) you wish to add using the check-box(es) to the left of your desired course sections, then press the submit courses checked button.
    Keep in mind: You can add multiple sections at once if all the sections you are adding have the same exam information and exam window(s). However, if the exam information and windows are different for your course sections, you will need to fill out a separate form for each section.
  5. On the Manage Requests page, locate the course that you just manually added. You can now continue with the "request proctored exam services for your course" instructions at step 6.

Adding or editing the alternate contact for your course

We strongly recommend having an alternate contact listed for your course exams, such as a TA or co-instructor. This helps to ensure a faster resolution in case any issues arise while your students are taking the exam at the Testing Center.

  1. Select Manage > Spring 2019 to go to the Manage Requests page.
  2. Locate the course for which you want to add/edit an alternate contact. Click the corresponding Course Actions button and select Edit Alternate Contact.
  3. In the window that appears you can add a new contact by pressing the + Add new contact button. Or, you can delete an existing contact that is no longer applicable by clicking the garbage bin icon to the right of the existing contact's information.
  4. If you added a contact, press the submit button. If you chose to delete a contact, you will need to confirm that you wish to delete the contact.

How to edit the exam information for an existing exam request

Please note that you will not be able to edit any of this information once the exam has been scheduled. If you wish to make any changes, you must do so as soon as possible after requesting the exam window. We strongly recommend contacting the Instructor Request Administrator by email in addition to any changes you make to an exam request in the Instructor Request tool. This ensures that your desired exam information and directions will be followed.

  1. Select Manage > Spring 2019 to go to the Manage Requests page.
  2. Locate the course and exam you wish to adjust. Click on the exam title to go to the Exam Details page.
  3. Click the Actions button and select the exam information you wish to adjust:
    • Exam Information contains the exam title, length of time, format, date window, and special instructions.
    • Exam Material gives the option to upload a reference/formula sheet if you want students to use one (make sure to specify this directive under exam aids)
    • Exam Aids provides clarification for what physical items you wish students to have access to, such as a calculator, scratch paper, etc.
    • Software refers to the software the student will need to complete their exam. This does not need to be adjusted unless you want students to use Microsoft Word or Excel to complete any part of the exam.
